The Short Answer

What a Nevada power of attorney for health care does

A Nevada power of attorney for health care lets an adult name an agent to make medical decisions for them if they become unable to give informed consent. Nevada allows co-agents and successor agents, and the person signing must sign it themselves.

To be valid in Nevada, the signature must be acknowledged before a notary or witnessed by two qualified adults who know the person signing. Unlike a real estate power of attorney, it is not recorded with the county; it is kept and shared with the agent, doctors and hospital.

As of 2026, TruPoint prepares it for $200, and our partner Nevada notary notarizes it by video for $25 per signature, so a typical total is $225. A durable financial power of attorney is also $200.

Nevada Signing Rules

Four rules that decide whether it is valid

General information about Nevada’s requirements as of October 2026. It is not advice about your situation; a Nevada attorney can answer questions about your own circumstances.

Signed by the person themselves

Nevada requires the person making the power of attorney to sign it. A family member can help with the device for a video signing, but cannot sign for them.

Notary or two qualified witnesses

The signature must be acknowledged before a notary or witnessed by two adults who know the person. We arrange the notary route by video, at $25 per signature.

Who cannot be the agent

Your own health care provider, or an operator or employee of a health care facility, generally cannot be named unless they are your spouse, legal guardian or next of kin.

Living in a care facility

If the person signing lives in certain care facilities, Nevada can require a certification of competency to be attached. Tell us at intake so we plan for it.

Questions People Ask

Nevada health care power of attorney questions

How much does a health care power of attorney cost in Nevada?
TruPoint prepares a Nevada power of attorney for health care for $200. Notarization by remote online notary is $25 per signature, so one person signing once is $225 in total. There is no county recording fee, because a health care power of attorney is not recorded.
My mother is getting forgetful. Can she still sign one?
The person signing has to understand what the document does and who they are choosing. Whether someone has that capacity is a medical and legal judgment we cannot make, so if you are unsure, ask her physician or a Nevada attorney first. If she can sign, TruPoint prepares the document for $200.
Does a Nevada health care power of attorney have to be notarized?
Nevada accepts either a notary’s acknowledgment or two qualified adult witnesses who know the person signing. TruPoint uses the notary route: we schedule a remote online notarization with our partner Nevada notary for $25 per signature, which avoids questions about who can serve as a witness. Preparation is $200.
Can my parent sign from home?
Yes. Our partner, a registered Nevada online notary, notarizes by video for $25 per signature, from anywhere in the United States. A family member can help set up the phone or computer, but your parent signs for themselves. The document itself is $200.
Can I name more than one person as my health care agent?
Yes. Nevada lets you name co-agents and successor agents who step in if your first choice cannot serve. Which people to name, and in what order, is your decision. TruPoint prepares the document with the agents you choose for $200.
Can my doctor or nurse be my health care agent?
Nevada generally does not allow you to name your own health care provider, or an operator or employee of a health care facility, unless that person is your spouse, legal guardian or next of kin. Tell us at intake who you want to name, and we prepare the document for $200.
Is it the same as a financial power of attorney?
No. The health care document covers medical decisions; a durable financial power of attorney covers money, bills and property. Many people sign both. TruPoint prepares each for $200.
Does it need to be recorded with the county?
No. It is kept with your records and shared with your agent, doctors and hospital. The power of attorney that usually records is one used for real estate, like a special power of attorney, which TruPoint prepares for $250. The health care document is $200.
When does the agent actually make decisions?
It is designed for the time you cannot give informed consent yourself. While you can, you make your own decisions. TruPoint prepares the document for $200 so your agent is ready if that time comes.
Our parent lives in a hospital or care facility. Can they still sign?
Often yes, but Nevada can require extra paperwork when the person signing lives in certain care facilities, such as a certification of competency from a physician or other listed professional. Tell us at intake where your parent lives so the document is prepared for the right signing. Preparation is $200.
Do I need a lawyer to make one?
Nevada does not require an attorney to prepare it. If you want advice about end-of-life choices, family disagreements or what to include, speak with a Nevada attorney. If you know who you want as your agent, TruPoint prepares the document for $200.
Can I change or cancel it later?
Yes. While you have capacity, you can revoke it or replace it with a new one. TruPoint prepares a new document for $200 whenever your choices change.
I signed one in another state. Is it valid in Nevada?
Nevada generally honors a power of attorney for health care that was signed in another state if it met that state’s signing rules at the time. Some people still prefer a Nevada document after moving here, and TruPoint prepares one for $200.
What information do you need from me?
Your full name and address, the names and contact details of your agent and any co-agents or successor agents, and any instructions you want included. That is all collected in the intake questionnaire. Preparation is $200.
Can you prepare one for both of my parents?
Yes. Each person needs their own document, so two parents means two documents at $200 each, and each signs at their own notarization, $25 per signature.
How fast can it be ready?
Drafting usually starts the same business day the signed contract and payment come back, and the remote notarization is scheduled as soon as the document is ready. Preparation is $200.
Do you speak Vietnamese?
Yes. Quinnie handles intake and signing instructions in English or Vietnamese, by phone, video or email, which helps when a parent is more comfortable in Vietnamese. The document is $200, prepared in English.
